Park Street Assisted Living Hub

The Park Street Assisted Living Hub is designed to help seniors manage activities of daily living in their own homes. SJHC staff provides non-medical personal support services to between 25 and 30 individuals who require access to care on a 24-hour basis in order to remain safe and healthy in their own homes.

Location: 155 Park Street South, Hamilton and the surrounding area.

Our on-site Program Manager coordinates and manages the services for the Park Street Assisted Living Hub clients.

Quality Services

The program is staffed by a team of Personal Support Workers (PSWs) with at least two PSWs on site at any given time, 24 hours a day seven days a week.

Services are provided on a scheduled and “on-call” basis to clients living in and around 155 Park Street South.  Clients can call at any time of the day or night for urgent or non-urgent support needs.  Our staff is able to respond in a timely fashion, travelling to the clients’ to provide service in the their own homes.

The type of service needed determines how long the visit will be.  Services offered include, but are not limited to:

  • Personal Support Services – Showering/bathing, dressing/undressing, personal hygiene, medication reminder visits or calls, toileting and other routine activities of daily living
  • Security checks and reassuring visits
  • Light homemaking services, laundry, and meal preparation
  • Preserving independence by assisting clients at their level of individual need and always promoting their right to self-direction

 

Referral Process

Referrals to the program are made through the Home and Community Care Support Services (HCCSS) Hamilton A HCCSS Care Coordinator conducts an assessment to determine eligibility for this program and, if eligible, a referral is sent to the 155 Park St. Assisted Living Program Manager. Once a referral is received, the Program Manager contacts the potential client to schedule a visit to confirm fit and provide more information about the program.

Eligibility:

  • Clients are able to direct and participate in their own care
  • 60 years of age or older
  • Require personal care assistance/support for daily living activities
  • Already living in 155 Park Street South, Hamilton or in the adjacent neighbourhood

Is there a fee?

Funding for this program is provided by Home and Community Care Support Services (HCCSS) and there is no cost to the clients for the services they receive.

All professional services (e.g. nursing) are provided through the Home and Community Care Support Services (HCCSS). Contact number for HCCSS is 1-800-450-8002.

The Program Manager and the HCCSS Care Coordinator work very closely together to ensure that all of the seniors' needs are met.
