First Place Supportive Housing

First Place Supportive Housing Program consists of 40 apartment units located in an apartment building managed by CityHousing Hamilton.  The on-site Support Services Manager manages and co-ordinates services for First Place Supportive Housing clients.  Support services are provided by Personal Support Workers who are on site 24 hours a day.

Apartments

Each tenant is provided with a one bedroom apartment consisting of a living room, bedroom, bathroom, kitchen, and balcony.

 

Location

The First Place Supportive Housing Program is wheelchair accessible and conveniently located at 360 King Street East at the corner of King Street East and Wellington Street North.

 

The First Place Supportive Housing program is just steps away from:

  • First Place Guardian Pharmacy
  • Europa Beauty Salon
  • Big Bee Convenience Store
  • HSR Bus Stops
  • Restaurants

Quality Services

The staff is in the building 24 hours a day and is available to help residents on a scheduled basis, or as needed.  This program provides a combination of personal support and homemaking services, security checks or reassurance services, and care coordination.

First Place Supportive Housing provides the following services:

  • Personal care including bathing, mouth and/or hair care, and preventative skin care
  • Personal support/ assistance: dressing/ undressing, assistance with pre-measured medication, exercising, reassurance and safety checks
  • Homemaking services: light housekeeping, laundry, and meal services
  • Crisis intervention
  • Information and referral
  • Advocacy

Referral Process

Eligibility for this program is as follows:

  • Clients are able to direct and participate in their own care
  • Require personal care assistance/support for daily living activities
  • Not need use of a mechanical lift
  • Require the need for 24-hour, on-site assistance
  • Be medically stable and not need intensive medical-type care
  • Have a phone
  • Require financial assistance for housing.

This program has been developed to provide patients from our area hospitals with permanent housing. Eligibility for the program is determined by the in Hamilton Niagara Haldimand Brant Home and Community Coordinator.

The First Place Supportive Housing Program Manager will review application forms and make arrangements to visit the applicant in the hospital to ensure needs can be met in our program.  Once approved, applicants’ names will be will be placed on wait list and offered an apartment when space becomes available.

Is there a fee?

There is no charge for the personal support services. Clients are responsible for their portion of monthly rent, based on a financial assessment completed by the Program Manager.

Professional Services

All professional services (e.g. nursing) are provided through the HNHB Home and Community Program call 1-800-523-8600.

The First Place Supportive Housing Program Manager and the CCAC Case Manager work very closely together to ensure that our clients’ needs are being met.
