Gwen Lee Supportive Housing

Gwen Lee Supportive Housing is located in an apartment building managed by CityHousing Hamilton. The on-site Support Services Manager manages and co-ordinates services for Gwen Lee clients. Support services are provided by Personal Support Workers who are on site 24 hours a day.

Apartments
Each tenant is provided with a private bachelor apartment consisting of a bed-sitting room, bathroom, kitchen, and balcony.

Location
The Gwen Lee Supportive Housing Program is wheelchair accessible and conveniently located at the corner of Main Street West and Hess Street in downtown Hamilton. Within the Main-Hess complex is:

  • A shopping mall with variety store, drug store, and other retailers
  • A seniors' recreation centre (Main-Hess Seniors' Centre) located across from the building at 181 Jackson St. West operated by the City of Hamilton.

The Main-Hess complex is close to:

  • HSR Bus Stops
  • Hess-Village filled with shops and restaurants
  • Jackson Square - a major shopping centre in the downtown core
  • City Hall
  • Art Gallery
  • Parks

Quality Services

The staff is in the building 24 hours a day and is available to help residents on a scheduled basis, or as needed. This program provides a combination of personal support and homemaking services, security checks or reassurance services, and care coordination. CityHousing Hamilton, a corporation that is separate from St Joseph’s Home Care, manages accommodation. The resident’s arrangements with the service provider are recorded in a plan of care; a lease governs the resident’s arrangements with the landlord.

Gwen Lee provides the following services:

  • Personal care including bathing, mouth and/or hair care, and preventative skin care
  • Personal support/ assistance: dressing/ undressing, assistance with pre-measured medication, exercising, reassurance and safety checks
  • Homemaking services: light housekeeping, laundry, and meal services
  • Information and referral
  • Advocacy

Referral Process

Eligibility for this program is as follows:

  • Clients are able to direct and participate in their own care
  • 60 years of age of older
  • Require personal care assistance/support for daily living activities
  • Require the need for 24-hour, on-site assistance
  • Capable of using an emergency response system
  • CityHousing Hamilton Eligibility

Is there a fee?
There is no charge for the personal support services.

Professional Services

All professional services (e.g. nursing) are provided through the Home and Community Care Support Services (HCCSS). Contact for HCCSS is 1-800-450-8002.

The Program Manager and the HCCSS Care Coordinator work very closely together to ensure that all the seniors' needs are met.
