As a member of the St. Joseph’s Health System, one of Canada’s largest corporations dedicated to health care, St Joseph's Home Care, along with five other healthcare organizations jointly share a President and Chief Executive Officer. 

At the local level, St. Joseph’s Home Care’s executive team reports to the Joint Boards of Governors, a volunteer board that governs the three Hamilton-based St. Joseph’s Health System Organizations (St. Joseph’s Healthcare Hamilton, St. Joseph’s Villa and St. Joseph’s Home Care).

Carrie Beltzner, Interim President

Carrie Beltzner is the Interim President of St. Joseph’s Home Care and Director of Integrated Care at St. Joseph’s Health System in Ontario Canada, leading a team focused on the expansion and implementation of the Integrated Comprehensive Care (ICC) program. 

Carrie has managed and provided oversight of a pay for performance program to support Emergency Departments across qualifying hospitals in the Province of Ontario. Her influence and experience have led her to hold various planning and implementation positions across Canada in leadership roles to execute complex change initiatives for health care providers and health system planning and oversight bodies.  She was also the acting lead for the development of the Province of Ontario’s strategy to support clinical adoption of a large provincial funding reform initiative, Quality Based Procedures. 

Carrie is a strong supporter of Addictions and Mental Health issues, serving as past Board Chair for a non-profit community-based addictions agency. Carrie holds an MBA from the Ivey Business School, is an Accredited Canadian Credit Union Director (ACCUD), and is currently serving as a Director on the Home Care Ontario Board and Vice Chair on the Board of First Ontario Credit Union.

Lori Lawson, Director of Community Support Services

Lori joined St. Joseph’s Home Care in 2014 from St. Joseph’s Healthcare Hamilton, where she held a number of progressive leadership positions.  In her most recent role at St. Joseph’s Healthcare Hamilton, Lori was a Discharge Specialist Project Lead for the Home First Refresh Project, as well as the Transitional Community Wellness Project.  Prior to that, she was the hospital lead for the Assess and Restore Program at St. Joseph’s Villa. 

Through her previous community engagement work, Lori has established strong relationships with funders and other service provider organizations across the Hamilton Niagara Haldimand Brant Local Health Integration Network.

Lori holds a Masters of Social Work Analysis of Clinical Practice degree, as well as a Bachelor of Art in Sociology and a Bachelor of Social Work, from McMaster University. 

Michelle Zivanovich, Director, Clinical Programs

Michelle is a Registered Nurse, and holds a BScN degree from the University of Victoria, along with a Canadian Nursing Association Certificate in Nephrology Nursing; she brings over 20 years of nursing experience to the St. Joseph's Home Care (SJHC) organization.  She has held several clinical leadership roles at St Joseph's Healthcare Hamilton in the Renal Program, including Charge Nurse-Independent Dialysis Unit.  As well, Michelle has worked on many Provincial Renal initiatives with the Ontario Renal Network (ORN)

Michelle transitioned to St Joseph’s Home Care in 2017. Since joining SJHC, Michelle has provided many progressive leadership roles including Clinical Resource Coach-Integrated Dialysis Care Program,  Sr Manager of the Integrated Comprehensive Care Program and currently as Director, Clinical Programs for the Visiting Nursing and Integrated Comprehensive Care Program (ICCP). Michelle is a member of the Centre for Integrated Care.

Michelle is passionate about professional practice, innovative models of care and patient advocacy and has been instrumental in implementing the Integrated Dialysis Care Program as well as expanding the ICC Program to include surgical streams, COVID Care @ Home and Virtual Care Models. . 

Michelle looks forward to continuing her studies in field of health management.

 
 Julie Turner, Director, Human Resources & Labour Relations 

Julie holds a Certified Human Resources Leader designation and has been a long-standing member of the Human Resources Professional Association of Ontario.  She obtained her undergrad BA from the University of Guelph, and her post-graduate masters in Human Resources Management from Seneca/York. Julie brings over 25 years of practical and progressive HR experience to the St. Joseph’s Home Care team, with in-depth Generalist knowledge and experience in the areas of labour law, occupational health & safety, organizational development, employee engagement, labour relations, compensation and more.  In the course of her career, Julie has held HR Leadership roles in a variety of industries including aerospace/project engineering, construction and transportation.  She has worked in both the non-profit and for-profit sectors, allowing her to bring a broad perspective to her work.

She started with St. Joseph’s Home Care in 2017 and currently works in the role of Director – Human Resources & Labour Relations leading all aspects of HR, Privacy, Risk, Health & Safety, and Information Technology Services. 

Julie is active in several provincial forums, working groups and System initiatives and believes strongly in providing mentorship and coaching opportunities to young and emerging professionals, thereby ensuring success for both the individual and the organization.  She cares deeply about positive work place culture and employee engagement.  She is excited about the future of Home Care and Healthcare in Ontario.
