The 2018 Annual Report to the Community is now available! This year's theme of "Serve, Innovate, Partner" encapsulates what we are about: serving clients, innovative programs and being a partner of choice. The report focuses on our achievement of Accredited with Exemplary Standing for the second consecutive Accreditation cycle, our Integrated Care Program – the first of its kind in the province of Ontario, process changes in our Visiting Nursing program to improve person-centred care, and the newly-launched First Place Supportive Housing Program. We are proud of how our employees are making a difference in the lives of so many in the Hamilton community and beyond.
