HAMILTON – St. Joseph’s Home Care is proud to announce it has achieved Accreditation with Exemplary Standing with Accreditation Canada.

This certification status milestone means that the organization exceeded the most rigorous requirements of the accreditation program.

“We are delighted with these results and our work continues. Quality improvement is a constant process of examining what we do well and find areas where we can do better,” said Jane Loncke, Interim President of St. Joseph’s Home Care. “It is by following this continual improvement process that St. Joseph’s Home Care can deliver consistent quality client-centred care.”

St. Joseph’s Home Care is accredited under the Qmentum accreditation program, which focuses quality and safety throughout all aspects of an organization`s services—from governance and leadership to direct care and infrastructure—to the benefit of patients, clients, residents, and staff.

SJHC was compliant with all 389 sections of the standard that apply to the organization, having successfully implemented every Required Organizational Practice (ROP)

“Accreditation is important to St. Joseph’s Home Care because it gives us an opportunity to examine everything we do each day from the lens of improving the quality of our services for our clients and residents, and providing a safe work environment for our staff,” said Ms. Loncke.

The Accreditation Report noted that, during the home visits, the surveyors found that SJHC nurses lived the mission of the Sisters of St. Joseph in their compassionate, respectful and professional relationships with patients. Other key observations included that the Integrated Comprehensive Care program, the Falls Prevention program and the hub and spoke for Assisted Living, are examples of how innovation and creativity in new program design are positively impacting services for frail and vulnerable patient populations across the continuum of health care.

Accreditation Canada is an independent, not-for-profit organization that consults with experts to develop health care standards based on best practices. They provide national and international health care organizations with an external peer review process to assess and improve the quality and safety of the services they provide to their patients and clients based on established and universally recognized standards of excellence.
