What the Help at Home Program Provides

Do you know what community services are available to help you or elderly parent(s) to cope with health or social issues while continuing to live independently? Our knowledgeable and trustworthy staff can assist you with the day-to-day activities in the care for you or your loved one.

Caregiver Relief

For those caring for their parents or elderly relatives, our staff will care for your loved one’s needs while you attend appointments, visit with friends, shop or enjoy time to yourself without worrying about the care and safety of your loved one. Nursing Services

Our nursing staff can provide clinical teaching to prevent illness and injury, alleviate suffering, and act as a health care advocate for clients and their families. Please click here for more information about our Nursing services.

Foot Care

Nurses can provide care in your home or arrangements can be made for a foot care nurse to organize a clinic at your care facility, retirement home, seniors centre or neighbourhood. SJHC also provides free foot care for people with a duel diagnosis of kidney disease and diabetes. Please click here for more information about our Foot Care services.

Home Maintenance

Designed to meet the specific needs of seniors and adults with disabilities living independently in their own homes or apartments, the Home Maintenance program is designed to provide services to help maintain your independence. Falls Prevention - Safety at Home

Our goal is to promote independence and mobility among older adults, allowing them to stay healthy, safe and strong by reducing the risks of falls at home. Available free of charge to anyone over 60 years of age, SJHC offers individualized assessments and tips to help prevent falls and injuries. Please click here for more information.

Care Coordination

Easy access to a range of community services is vital for allowing you or your elderly parent to cope with health or social issues while continuing to live independently. Hospital to Home - Care After Discharge Program

Our "Hospital to Home" program includes:

  • Accompaniment from Hospital to Home
  • Stops on the way home for groceries and prescriptions
  • Meal preparation
  • Help getting settled comfortably
  • A phone call to inform you that your loved one is safe at home
  • Care Coordination
