Neighbourhood Model for Seniors at Risk

The Neighbourhood Model for Seniors at risk is a care service provided to seniors who live in their homes within the designated geographic area bounded by Wellington Street, Victoria Avenue, Cannon Street and King Street.

Location: Main service hub is located at 226 Rebecca Street. This is an apartment building managed by CityHousing Hamilton

Our on-site Program Manager coordinates and manages the services for the Neighbourhood Model for Seniors at Risk clients.

Quality Services

A team of on-site Personal Support Workers and home cleaners are in the building Monday to Friday 7 a.m. to 7 p.m., and are available to help clients on a scheduled or as-needed basis.

The Neighbourhood Model provides the following services:

  • Personal care including bathing, mouth and/or hair care, and preventative skin care
  • Personal support/assistance: dressing/undressing, assistance with pre-measured medication, exercising, reassurance and safety checks
  • Homemaking services including light housekeeping, laundry, and meal services
  • Information and referral
  • Advocacy
  • Social and wellness promotion events

Referral Process

Eligibility:

  • Clients are able to direct and participate in their own care
  • 60 years of age or older
  • Require personal care assistance/support for daily living activities
  • Already living at 226 Rebecca Street & 360 King Street East or in the adjacent neighbourhood

Is there a fee?
There is no charge for the personal support services.

Professional Services
All professional services (e.g. nursing) are provided through the Home and Community Care Support Services (HCCSS). Contact number for HCCSS is 1-800-450-8002.

The Program Manager and the HCCSS Care Coordinator work very closely together to ensure that all of the seniors' needs are met.
