On March 19th 2022, the Integrated Comprehensive Care (ICC) Program celebrates a
decade of delivering integrated and evidence-based care.

The 2021 Annual Report to the Community is now available!

The Integrated Comprehensive Care (ICC) Program has been successfully operating within St. Joseph’s Health System (SJHS) and Niagara Health (NH) since 2012 for Thoracic, Cardiovascular, Hip and Knee replacement surgery as well as patients with Chronic disease (COPD and CHF).

St. Joseph’s Home Care (SJHC) launched COVID Care @ Home in November 2020, and supported referrals until June 4^th, 2021
